Considering opting out?
If you have been charged for the Students’ Association’s mandatory health and dental plan and have existing coverage you can opt-out of our plan with proof of your other coverage. All you need to do is fill out the waiver form here. You will be required to submit an image or scan of your existing coverage during this process. The deadline to complete your opt-out is Fri, Sep 11 at 4 pm.
Please keep the opt-out confirmation email that you receive as proof of your opt-out.
Want to add family members to your plan?
If you have family members that you would like to add to your plan you can do that by completing the add-on form here and paying the required additional fees. Once you’ve submitted the add-on form, a SANQC representative will be in touch with to provide further details on how to pay the add-on fees. The deadline to complete your add-on is Fri, Sep 11 at 4 pm.
One additional member: $579 for both health and dental, $289 for one of health or dental.
Two or more members: $965 for both health and dental, $482 for one of health or dental.
Note: You may only opt-out/add-on during this period if you have had Health & Dental charges applied to your account starting in the Fall term.
Did you previously opt-out but now need coverage?
Students that previously opted out can re-enroll in the health and dental plan on the anniversary of their opt-out or within 30 days of loss of coverage.
