Considering opting out?
If you have been charged for the Students’ Association’s mandatory health and dental plan and have existing coverage you can opt-out of our plan with proof of other coverage. All you need to do is fill out the waiver form online or in person at the SANQC Office (1-114) and show us your coverage. The deadline to complete your opt-out is Thu, Sep 13 at 4 pm.
Want to add family members to your plan?
If you have family members that you would like to add to your plan you can do that in the SANQC Office (1-114) by completing the add-on form and paying the additional fees (cash only).
One additional member: $579 for both health and dental, $289 for one of health or dental.
Two or more members: $965 for both health and dental, $482 for one of health or dental.
Did you previously opt-out but now need coverage?
Students that previously opted out can re-enroll in the health and dental plan on the anniversary of their opt-out or within 30 days of loss of coverage.
